Is it possible to change the shipping rate of an order that's been placed but not yet fulfiled? A customer missed or forgot to check the express option. The only thing I'm able to change on the order are the items in it.
Or would the best action be to have them cancel their order and create a new one chosing express?

You can't change the rate of a completed order. One potential option is to edit the order and add a custom product to the order called "Upgrade shipping to Express" with the price entered for the difference in shipping cost. It will then allow you to invoice the customer for the difference.
