Based on what you have shown, everything does appear to be in order. I have gone ahead and attempted a test order using the information in this screenshot here. This does now appear to be working for me:
I am thinking that in this case it may then be relating to your browser cache, and previous shipping info being stored by mistake. Would you be able to clear your cache or test this out using a different browser?

I had thought the rates had been specified - my mistake. Though in either case I am glad it is working now for you.
