POD Returns / Exchange Policy

Hello, I am creating a new store, and using print on demand.

For any POD'ers out there, I'd appreciate your input on these questions:

1. Do you charge your customers shipping in order to cover your printer's shipping costs? Or do you factor shipping into your product price?

2. How do you handle exchanges and returns without significant loss?

3. Have any of you had any experience with Printify, Printful, or CustomCat?
