I looked into this for you and found a couple of apps that could work for you here. They aren't specifically box ticks apps but more extra product fields given customers a chance to fill in more details. The two I found can be seen below:
If neither of these apps can do what you're looking for, your best bet would be to have something custom made by a Shopify Expert. You can see the Shopify Expert directory for custom features and code here.

If you are wanting to add tick boxes (we call them checkboxes) to your store's registration form, then I suggest taking a look at our Customer Fields app: https://apps.shopify.com/customr
The Customer Fields app allows you to easily collect extra information from new customers when they register for an account. The app also includes an "Edit Account" page which allows existing customers to login and update their account details.

I need a checkbox form at the checkout or straight after checkout.
I could use the shipping form for this purpose and tweak it to my purpose actually, but I also
need the shipping form.
I was thinking / hoping that there would be a form that i have not discovered yet in Shopify checkout that
I could use as a check box or a form where my clients fills in their specified wants and needs when it comes to the product they
just bought.

Unless you're on Shopify Plus there really isn't an easy way to edit the forms that are shown on the store's checkout process.
Since it sounds like you're looking to collect information that is related to the specific products a customer is ordering, I am not sure if our Customer Fields app would be the best fit. That being said, I've heard good things about Bold's Product Options app which would allow you to add checkboxes to the product page(s): https://apps.shopify.com/product-options
