Sesami date and time boxes missing on purchase

I have set everything up according to the instruction video from Sesami. The red outlined boxes to pick a date and time for the service purchased (I’m doing a test on my live store) do not appear.

There is no way for a customer to pick a date and time. Pay, yes, but the critical step of scheduling is completely absent.

It's Karmel from Sesami. 

Mostly, It's a theme issue and need some tweaking to have it works as it's suppose to be, Please submit a support ticket here and i'll make sure everything works fine within 24h. Also, Could you please share your store's URL here, So i can accelerate this for you?

Hello,
I did not understand how the theme worked — the way the Home Page functionality differed from the Catalog page. When Sesami’s folks explained it, then I was able to adjust the information and now it I think everything works beautifully. Excellent support.
