advanced form - sending product information to me

I need an advanced form so my customer could sent me all information about products they want to sell throuth my ecommerce platform (including upload section).

Any advise for an app?

I looked at many apps, but for now I didn't find what could fit for me!

It appears that site is using the Go-Daddy web builder. I have used it before.
