Great question! The one hour support time is for any store owners who are using a Shopify developed theme and have a paid plan. Something to note is that the one hour design time isn't for major customizations but small task changes within your theme.

Great question, but once the hour is up it's not possible to purchase any more time from the Shopify theme support team. You can see more about this from the Shopify design policy here.
In saying that, your best bet would be to reach out to a Shopify expert about this. You can see the Shopify expert directory here.

The 1 hour free design time would only be available for a Shopify developed theme. You mentioned you are using the Streamline theme which is a paid third party developed theme, I would suggest starting with the theme developer for any changes and then a Shopify Expert if the developer cannot assist.
