Set up a product customizer funnel.

We're setting up a site offering pet treatments and we require the user to tell us some more information about their animal, such as, if the pet is a Dog or a Cat, their weight, if they have health issues and a few other aspects in order to offer them the right product. We want to set up like a funnel or user journey where the user can answer these questions in various "Steps".

An example similar to what we want to achieve is: https://www.itchpet.com/signup/pet-name

I am looking for recommendations of how I can achieve that and what apps we should use for it.
