Hi there. I'm trying to figure out how to create a form in the product page like this https://www.framedbysarah.com/product/monogram-box-product/. I'm new to Shopify and I'm not sure if I need an app for this or if I'm missing it somewhere. TIA

You can do this without the need of an app but you would need to custom code this. You can try using this link: https://ui-elements-generator.myshopify.com/pages/cart-attribute to guide you in your creation.
