Custom fields on check-out page

Is it possible to add custom fields (both required and optional) to the check-out page? (not the cart page)
I need to add: Delivery date, phone number, email address, and an optional note.

Only on the cart page. the checkout is off limits due to PCI compliance.

No. It is not possible to put your stuff on the checkout page. Use the cart page, that is what it is for in this case. 

Collecting the extra info like delivery date, phone number, email and note can be grouped nice in a fieldset, and to be deluxe you could collect one per product too...

I need to add an extra field to the check-out page. I'm only using the Shopify button and integrate that with a webflow website. So people can add items from the site and then the cart icon appears. That directs to the check-out page (see below). On this page I want to add an extra field. Is that possible?
