Customer Getting a Charge

So yesterday I gave one of my customers a discount code (100% off she just had to pay shipping) & today she messaged me & said she got charged the full amount. There is nothing on my end that shows that though. I only see she paid the shipping cost. She sent proof & everything. I told her it may be something on her bank's side, but wanted to see if this has happened to anyone else, or maybe what caused this?
