Staff account access

I have created a Staff account. They get the invite, register with their work email as required and then get prompted to ask for a permission. Permission gets sent automatically from their personal email and I get blank email sent to me.

We have tried to remove her account, added again, but same thing all over again. 

Not sure where and how to proceed from here. 

Any feedback appreciated.
