I wanted to include a 'how did you hear about us' form with some options and can't seem to get it to capture. Is it supposed to show up after the purchase is made? We have not officially launched yet, and no one has bought out product.
So the two part question is... is it working? And when is it supposed to prompt our customers?
Any help of this would be great.

Here's a 2 simple ways to do that:
Redirecting after a successful purchase : Create a for on google forms with your specific questions ( try to make it short ) or use one of shopify apps that helps you build forms inside your website, and create a new page and frame that form on this page after customising it and make it looks great, now save it and go to setting > Checkout > Additional scripts then add the following script

change thehttp://YOURURLHERE.COM with your created page
then try it if it's work well
Email Automation: Use your transactional emails which is the highest opened emails ( 81% open rate ) and add the form there
