Looking for a shopify guru

Hello,

We are a B2B wholesale store in the healthcare industry. We currently have over 150 products in our store and many more to come. We serve certified and licensed healthcare providers across the country managing their disposable medical supply needs. We are in the last phase before launching and there are 2 components that need completion.

1) The first is a new member application form. Our prospective clients will fill out the fields with the required information and submit to forward to our inbox for review. All pages in the store concerning products, search bar and pricing are locked until the customer is verified and is added as a member.  If approved the customer profile is created and can obtain credentials to log in and shop. Here is the a good example of the form https://americangpo.com/join-ahp/. I have the exact content of the the form on a word file.

2) We are looking to add a widget and add a payment option to our B2B customers. This will allow them to apply for flexible payment terms as help provide finance options to customer. This lender is 3rd party lender that can be used with the shopify platform. You can find some background on the company here https://www.behalf.com/behalf-for-ecommerce/. Instructions on how to add the widget are here and how to accomplish this here https://developer.behalf.com/docs/qualification-widget. We need the widget in strategically place in the store and gain a good understanding of the checkout flow.
