How to add card upsell and personal note option at checkout?

Hi all

I am wanting to sell baby gift boxes that contain a variety of baby gifts ranging from shoes to small toys.  Upon checkout, i want there to be an option so that the customer can pick variety of gift card messages (ie like a birthday card) and then a 'note' section where they can input their message - we will physically write the message for them... See below example, this is from another site which we want to have this option also.

Is this possible via the native shopify store or do i need a 3rd party app?  Any direction would be greatly appreciated.

Hi.

Unless you are on the plus plan, this would not be likely in the checkout area.

Though a suggestion... once the user clicks "Checkout" on the cart page.
Create a new page that the user ends up at, then you can customize that page as much as you want.
Then just add any new items to the cart and send them to checkout.
Checkout will reflect the new items.

We can create a dropdown for selecting the Card option and give a textarea to users so that they can add their text and we can save the data with order from the cart page,

We don't need any app for it.
