Attempt to install app resulted in redirect to admin with oauth_error=problem_with_request in URL

Hi,

We had a Shopify store that encountered this error (see screenshot) while trying to install our payment app from https://apps.shopify.com/pay-by-bank

Note, this is not the common redirect URL whitelist issue.

- other merchants have been able to install our app both before and after this incident occurred

- our server logs show a request was sent without any shop domain info around the time this error was shown in Shopify admin

Has anyone else encountered this?

There was an outage yesterday involving the installation of apps. This has now been resolved.
