Customer Cart notes not showing up in the Order object

Our store suddenly stopped populating the order notes that customers input in the cart page to the order object about 2 months ago. We haven't installed any new apps for months before then, or made changes to the cart file or settings. The Notes section of the Orders are empty and the order object notes attribute is NULL.

 

Any ideas for how to fix this? The checkout -> order creation is a Shopify black box so we're not sure how to debug this.

Would you have installed any apps that could have changed the native order notes feature?

No, that's what's puzzling. We haven't added any new apps since Oct, and don't have anything that impacts the cart. We do have a second custom notes input section in the same place, but that's also always been there.

If you remove the additional custom notes section, does the first one work?
