Best quiz to get monthly box subscriber data and push to Klaviyo for personalised emails

I have a subscription box store, and I want to put a quiz on the front end so I can personalise/segment my emails.

• Is there an app that we do it post purchase in the user's Shopify account once they purchase? Or does this require coding? 
• Or, does it need to be a lead magnet of sorts like Bark Box? And set up the quiz pre-purchase
• Or, how can we get that data another way that will feed into Klaviyo for us to achieve the goal?
