How do I send an automatic email after a customer purchases a specific item from my store?

I have a product that can be upgraded and I need to send an email to customers after their purchase is complete to give further instructions.

For example: A customer buys a pair of shoes, a scarf, and a hat. The shoes can be upgraded and hand painted. So, even thought they purchased other items, the shoes were purchased and therefore I want the hand painting instructions to be emailed to that customer.

You should just be able to just set up an automation to trigger after a customer buys a specific product.

I just checked, and it looks like Shopify Email/Marketing Automations doesn't support this in their default capabilities, but you should be able to use another app that supports granular email automation triggers like this.

The action you want is possible with email marketing apps that provide Automation. And like Popsmash mentioned, I found the Shopify email app didn't support that. But there're plenty other apps on the App market can help you with that. You can check on some popular names like Klaviyo, Omnisend, Avada...

The trigger can be Placed order and you may add additional condition like the Product that they buy.

We use Klaviyo and can send emails to people who purchase a particular product quite easily. Just need to setup a segment list that says all people who purchased product A.
