Reseller Ratings - Where is the Thank You Page?

I'm attempting to set up the pop up survey after checkout for my Reseller Ratings account, but I cannot find the Thank You Page to complete the code snippet that needs to be added in my settings page (according to the instructions from Reseller Ratings).

Has anyone actually ever successfully added that pop up survey?

Where the heck is the Thank You page anyway?

No, I am already a subscriber/member of Reseller Ratings which is what I want to implement. I don't want an ad for a different Shopify App.
