Which feedback form can I integrate for store incentives?

Does anyone have a feedback form reco that I can easily add to my store?  I want to tie it to an enter to win action as incentive.  Thank you!

Thanks very much Daniel.  My business is DTC custom art built via tech back end.  We are still in release 2 of our beta so I want to take advantage of the people visiting my site and bouncing to find out why / what would keep them.  Any advice is greatly appreciated. www.lagomcollections.com

Upon visiting your store, please refer to the following: 

• What a creative product idea. I can definitely see bookworms and literature fans being very excited about this kind of art product. 
• I also enjoyed the colors you have chosen for your store. It pops immediately upon entering the site. 
• Adding some product reviews or testimonials is a proven way to increase those conversions. Buyers like to see what other buyers before them think of any product. 
• I was going to suggest adding a blog but upon further inspection, The Stacks section seems to be one on your site. This is a great way to further market your products but I would suggest making it clearer for those customers who are not familiar with your site. Maybe you can change the navigation menu to inform customers this section is a blog. If I didn't click through your site, I would not know that "The Stacks", is a blog section.

• It also seems you are using a Shopify-made theme. If so, please visit this link as it goes over our design policy where our themes team can assist you with some possible customizations.
