Why isn't my Buy X Get Y discount functioning properly?

I have a promotion going on today - Spend $50 Get a $10 GC, Spend $100 Get a $25 Gift Card, Spend $200 Get a $25 Gift Card.

I have three separate discounts active - one for each of those deals. It was tested this morning and working, added a line item for the free gift card to the cart automatically. But I noticed it was adding multiple gift cards based on the spend so I set it not to combine. After I did that, it stopped working altogether. I set it to combine again, deleted the discount and made a new one, etc. Now no gift cards are adding to the qualifying carts and I have to issue manually with each order that comes in.

What else is there to try? There are only so many buttons to click when creating the discount and I've double checked all the options are correct and make sense for what it needs to work!
