I need to add chargable personalization fields to all the products in my inventory

I just opened a pop up store for some school merchandise. I need to be able to have my customer select 2 options in addition to the standard size and color.  1, if they want their last name on the item ($5 upcharge associated with it) and there will have to be a field to collect their name. 2. if they want their number on the sleeve ($3 upcharge assoicated with it) and there will have to be a field to collect their number

Hi @Shawng1 👋 if your not using all 3 variant productOptions , then this can be done with with advanced theme customizations that reveals a field when the 3rd option "embroidery", etc is selected.

I'm Marton from EasyFlow Product Options. You can do this customization with our app easily, here are the steps after installing the app on this link:

1. Create an option set eg. Customize School Merchandise 
2. Create 2 "Check box" options within the options set (eg. "Add last name" and "Add number on sleeve") then add an add-on price to them:

3. Once you are done with the 2 checkbox options, create 2 "Text box" options, one for "Last name" field, and one for "Number on sleeve" field

3. Click "Create a rule" within the option set. Set 2 rules: 1) Select the "Add last name" option, "is one of", "all values" Then "show option", "last name". 2) Select "Add number on sleeve","is one of", "all values" Then "show option", "number on sleeve"

4. Save the whole option set and on the right add it to the products you want. 
5. Once it is saved, and the app block is added to your online store, it should look like this on the product page:
