Custom Store Pages

Hi, this is my first time posting a question on Shopify!

I just launched my store about 2 weeks ago. I'm looking to add more website options/pages. 1 is a custom contact form. I have used the contact form template but there is a limited amount of fields to fill out. 

For example, I want to set up a business wholesale form for my coffee company. How can I include fields such as "business name" "estimated # of bags", etc. 

How can I get this accomplished?
