Where can I find the nft-application form?

HI, 

We are building a App that requires the store to apply to be eligible for NFT sales. In the documentation it says we should give the store the following link to a form that would make the store eligible to sell NFTs (That we create for them) once filled out: https://{shop}.myshopify.com/admin/nft-application as seen here https://shopify.dev/docs/apps/blockchain/nft-distribution.

That nft-application link goes to the 404 not found page on test and live stores.
Where can I find the form? Is it still applicable for selling NFTs?

Reviewing you post here. it sounds like you are seeing unexpected errors returned when loading this URL link/page in the admin. When we tested in our development store using the docs, this worked as expected. If you have already; 

1. confirmed partner eligibility requirements and been accepted to the blockchain app program 
2. checked merchant eligibility via API  
3. worked through local troubleshooting such as clearing your browser cache or using an incognito tab 

Then, the best next step moving forward would be connecting with Shopify Support directly so someone can take a closer look.
