Calculated ship rates not showing for customers

We have one flat rate custom rate for domestic, AND a regular first class package or priority mail choice.

When customer check out for one 2oz product, it defaults to offer them ONLY priority mail.

This is not correct, it used to work and now it is not, I need help.
