Customer got free shipping without entering my code

A customer of mine somehow got free shipping along with entering one of my %10 discount codes. She never entered my free shipping code and my discounts are not set to combined. Anyone else have this issue or know how I can fix it? I don't want them to get free shipping along with a discount code.
