How do I enable "cash payment" in my store?

I follow the step to add "manual payment" (which I name it like 'paid on pickup'),

but I cannot see this option in the checkout page. how do I enable this manual payment in my store? Thanks.

Thanks for detail information and steps. I almost do the same thing as yours.

I finally figure that out, I install a purchase option ("pre-order"), and that allows credit-card payment only. and my manual payment option become available once I disable that app.
