How to get Shopify Shipping app to recognize Uline Thermal 4x6 Labels template?

I purchased Uline Thermal 4x6 Labels (A6) for shipping Shopify Shipping Template..

I am having trouble getting the template to work with the shipping app on the site.

I was trying to get a hold of Shopify Chat- but I see that is down??

Does anyone know how to get the Shopify Shipping permissions to recognize the template?
