Manual payment / bank transfer confirmation

Hi! I'm trying to add payment confirmation form to checkout page for customers who chose to pay by bank transfer (Thailand). Here's a good example of what I'm looking for.

It may be possible to add this on your Shopify Store Thank You and Order Status pages using a Checkout UI Extension but I don't think there are currently any Shopify apps that let you do this at the moment. But I did recently set up a workaround for another merchant using the Checkout Blocks Shopify App and the Powerful Form Builder Shopify App.
