Online Order Form - Don't know what customer is attempting to order

I have an online order form. Whenever a customer submits an order, I get the email notification below. The issue is that I do not know what the customer ordered because this email notification does not include any style description. When I login to shopify, there is no record of the order.  My order form is also below for your reference.
