Why is the discount code box not showing in my checkout process?

Hi there!

This seems to be a common issue, but the discount box in the checkout portion of my store disappeared.

A customer brought it to my attention recently, although, I've had previous customers use my discount code for free shipping in the past successfully.  I'd love to get some help bringing this back on my site!
