How can I smoothly collect customer information for package labels?

Hello guys

My name is kristian and im from denmark trying to make my webshop 🙂

At my store we sell candy and stuff like that.

but we also sells something called "pakkelabel" or package label

And then i need a lot of information from the costumer, but how can i make it more smooth than people need to send me an email with the informations

i need:

Name

Package size

personal adress

adress they need to send it to
