404 redirects and Smart SEO app

Hello, I recently downloaded the Smart SEO app to my business' Shopify site. https://apps.shopify.com/smart-seo for app reference. 

Issue I'm having is the page I'm trying to redirect is not going to the desired URL; screenshot below. 

Does this need the full URL posted in the "repaired URL" column? Add that is hasn't had this problem with other pages I've redirected since installing this app.
