Allowing personalization add on

Hi,

I am looking for an app that will allow me to add an option to allow customers to personalize their product.

We sell snacks and can create custom labels for the bags they come in. I do not need a preview as they will work with our graphic designer - just looking for something I can have to allow them to purchase that option. I have added in a screen shot to give somewhat of an idea of how I would like it to look.

If you just want to like give users an option to add an image that you can see in your orders section, you might not necessarily need an app but yes since you are charging extra, that is something code does not offer that now and i will need to look into it.
