APP disappeared from my app list and stopped working but the add to cart button and other items site

Hello,

I installed an 'upload' APP and it has been working fine for a few months, i recently got 2 orders but the uploads didn't come through correctly with the images,

So i checked my apps list and it was no longer on the list, however the app features are still on my site and people are trying to upload to them, how do i delete the app features from my site as i dont want them there as they no longer work,
