App for letting customers choose a delivery date based on their delivery area

We are hoping to allow customers to choose a delivery date based on their shipping area after entering their address. We deliver locally, but due to an app we use for delivery zones (Mapit) we have to use Shipping as the fullfillment method on the backend. We have three main delivery zones and deliver to each on different days. Our hope is that a customer will be able to enter their address, and be shown the possible delivery days for their zone which they could select.

You can display a list of possible delivery days at checkout for a given delivery address (zone), using our app JsRates with custom rules.
