App recommendation that allows two checkboxes at checkout?

Hi all,

I need two checkboxes at checkout. One for the Terms and Conditions/Privacy Policy and a second separate checkbox + custom text right below it. I see a bunch of apps for adding one checkbox, but I can't find any that allow a second checkbox to be added. Any recommendations?

Hey!
The terms and conditions check box can be required to be checked before proceeding. Other checkboxes cannot be as of now, but if you need then we can help you with making them required.
