App that allows customers to fill out text fields to customise product and checkout

Hello,

Right now I am creating a store page where I would offer customers to customise a product like a ticket by choosing style and filling out text fields, which after  pressing add to cart would get saved and I could print that out. 

What I am looking for exactly is an app which allows to fill out several custom text fields and either the best option would be that it automatically live updates on the product image or it would also work that it just saves it and sends to me the info with inputted text for processing. 

I have tried several apps but none of those seem to offer this solution. Could anyone suggest something that offers these possibilities?

Bellow I have added a screenshot of the desired outcome idea. Left side is where customer inputs text and the right side is the image where the fields correspond and update live on the image.

Your either looking for a personalization app with image-preview or image generation capability.

Or a contact form app ala shopify-forms https://apps.shopify.com/shopify-forms 

Or advanced theme customizations to customize the contact form, and or overlay text output on a static background image.
