App that books appointment of a full cart at checkout?

Hey there,

I'm looking for an app that books appointments, but we want it so that it appears during checkout.  We're going to offer multiple products for rental, but all of the apps currently prompt the scheduling right at the individual item level.  We need to build a full cart and then at checkout have the scheduling take place.

On the other hand, we would be open to an app that shows a schedule first and then you add items to the cart after that.

I've test ran a few and had no success.  Any ideas?
