App with an initial fee and a reoccurring subscription

I have a pooper scooper business and I am working on the website. I need to be able to charge people weekly for the subscription service, but they also need to be charged extra on the first clean up depending on how saturated their yard is. I want it to go something like this...

Yard Saturation | Light | Medium | Heavy

Number of Dogs | 1-2 | 3-5 | 6-8

and we need to vary the initial price based on "Yard Saturation" but the number of dogs options is a seperate calculated price.. Any ideas?

Do you use any 3rd party solutions to manage subscriptions?
It would be great to hear more details. But in general, you can use a third-party app and extend its functionality or create a custom solution to meet your needs.
