Appointment Date/Time picker that updates based on location?

My company has several store locations, each with different days/hours of operation. On our "Schedule an Appointment" form, we need to implement a date/time picker that updates to reflect the available days and appointment time slots based on the store location you select. Right now, we have a basic calendar date picker that only shows available days, not time slots, and we cannot set different days/hours of operation for each location. 

We are currently on WordPress (planning to launch the new Shopify store by the end of the month) and we have this functionality through the InputWP plugin. Looking for a Shopify app equivalent.
