Cart Order Form

Hi, I am looking for an app that will allow me to add a custom order form to my cart to follow a specific flow for example.

Customer add product to card --> Click the cart and clicks check out --> goes to an inline form that asks a yes or no logic question then goes to another form to input specific address data --> once the flow is complete the last button is the shopify checkout button to go to checkout.
