Checkmark options not displaying (Infinite Options)

I'm using Infinite Options and the checkmark options are not displaying. However, when I preview the matching product on Infinite Options, it shows.

Help! 

PS: The preview matching product is also unappealing. The checkmarks are in a single line rather than spaced. How can I change this?
