Contact form with upload field

Is there any contact form app with support for upload fields that is completely free? I've tried a couple of apps but they only offer 7-days trials. I don't mind any monthly submission limits as the form I need to add to my shop will be used sparingly.

I did a bit of research into this and I believe I found a solution for you using a third party integration. I took a look at Cognito Forms, and then have a free plan available that supports file uploads. You can then use this embedding guide to copy and paste the html in your pages. Since you plan to use the form sparingly, the Individual (free) plan should work well for you!

I'm curious, what do you require your customers to upload in the contact form?

As it's right around the corner, have you thought about how you're going to market your store for Black Friday/Cyber Monday? I'd recommend taking a look at our BFCM toolbox for some ideas about how to make the most out of this busy shopping season!

Thanks! I will take a look at them. Nevertheless, it would be cool if your standard contact form supported file fields by default.

The store has a "Work With Us" page for job seekers to submit a resume for consideration
