Converting Offline Purchases to Online Rewards

Hi,

Is there a loyalty app that has the option for a customer to register or upload an offline purchase made from a grocery store and earn points for that purchase?

The idea is to have a link or a QR code on the product label that encourages the shopper to scan to create an account on our website and then register the purchase to earn certain rewards.
