As Smile: Loyalty & Rewards is considered to be a third-party app, you will want to connect with them directly. This will allow the app developers to take a closer look into that documentation and provide some clarification on its limitations.
To connect with the app developer, you can head over to the Shopify App Store and scroll down to the 'Built by Smile.io' section to view their support options.

I asked them this and they said yes they do every order processed through Shopify counts. This includes Shopify POS orders that don't have a customer assigned to them.
