Experiencing issues with the product review app on Shopify?

has anyone had trouble with this app before?

none of my reviews are showing only the option to leave a review is showing, when i click the link for eave a review it just refreshes the page. i havent changed anything in the app and it was working fine a couple of days ago.

any help is greatly appreciated.
