Find My Local - APP

Hi I wondered if anyone know of a ready made app - Basically I want a page on my website for my customers to find there local PT (Personal Trainer) and there credentials also i would want the customers to be able to book the PT through my Page and I get a copy of the Request as eventually I will be Charging the PT any help would be amazing I'm new to shopify and still learning.
