From the store admin Billing settings (admin/settings/billing/subscriptions), when viewing active subscriptions. Select an App and click the "Manage" button under "..." on the right for the app. It appears like the App URL listed in the App Setup is being launched without the HMAC, shop, and other URL parameters that are used to authenticate the request.
This results in the page not being loaded as the App backend cannot authenticate the visit (from a specific store).
Has anyone run into this issue? Is the App required to do something special to handle this method ("Manage") of launching the app? The launch from the "Apps" seems to provide all the parameters correctly.
